If your student requires access to medication during BAC program hours, please use the following guidelines to drop off medication at the Rec & Ed Admin Office (1515 S. Seventh St. behind Pioneer High School) prior to the first day of school.
1. Physician signature on Medical Authorization Form (or equivalent form) (see below).
2. Parent Signature on Action Plan (see below).
3. All prescription medications are in original boxes with prescription form attached.
4. All over-the-counter medications are in original boxes with the student's name on container.
5. All medications are NOT expired.
6. Everything is in a gallon zip-top bag with student name and school on it.
